Faith Ambassadors

Advance care planning is a way to help our loved ones make medical decisions for us when we are not able to make them ourselves. This can be a daunting topic. 60% of people say that this communication is extremely important, yet 56% of the same individuals have not documented their preferences.

Faith Ambassadors are trained to make this easier. We can help with the process as well as assist with getting the needed form filled out correctly.
